Job Description:

A company is looking for a Bilingual LPN Care Navigator.
Key Responsibilities

Engage in monthly telephonic patient interactions to support care goals
Create individualized care plans and manage coordination via proprietary software
Document interactions and escalate clinical concerns as necessary

Required Qualifications

Bilingual (Spanish/English) - fluent in both written and spoken communication
Active Licensed Practical Nurse or Licensed Vocational Nurse license with multi-state compact license
Minimum 2 years' clinical experience
Experience working with EMR systems and familiarity with chronic condition management
Intermediate to advanced computer skills and telephonic communication skills
