Travel Job Description Summary: Seeking an experienced Home Health
Registered Nurse for a 13-week travel assignment based out of
Torrance, CA. This RN will deliver high-quality skilled nursing
care to patients in their homes, servicing the South Bay, Long
Beach, and nearby areas. The ideal candidate must be phlebotomy/IV
certified and experienced in managing a mobile caseload using OASIS
documentation. Responsibilities: Provide skilled nursing care to
patients in a home health setting. Conduct assessments and document
in OASIS and Epic EMR systems. Administer medications, wound care,
phlebotomy, and IV therapy as needed. Travel between patient homes
with a standard of up to 6 visits per day (30 productivity points).
Collaborate with care team members to coordinate and implement care
plans. Ensure compliance with infection control, safety, and
quality standards. Maintain accurate, timely clinical
documentation. Key Requirements: Minimum 1 year of recent skilled
Home Health RN experience. Must be IV and phlebotomy certified.
OASIS documentation experience required (must be noted if candidate
lacks this). Must have own reliable transportation; mileage
reimbursed at IRS standard rate. Maximum driving radius
occasionally may exceed 50 miles. Facility groups clients to
minimize windshield time. COVID vaccine primary series one booster
required. RTO must be confirmed with hiring manager. First-time
travelers not accepted. Qualifications Education: Graduate of an
accredited School of Nursing Experience: 1 year of recent Home
Health experience (skilled nursing) License: Active California RN
license Skills: Skilled nursing in home health setting OASIS
charting experience IV therapy and phlebotomy Strong documentation
and time management Patient assessment and individualized care
planning Proficiency with Epic and Oasis EMRs Effective
