Job Description LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Burbank, California. Job Description & Requirements - Specialty: Case Management - Discipline: RN - Start Date: 07/21/2025 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel - Minimum 2 years acute medical care management experience in care coordination and discharge planning - Proficient in Epic charting system - Travel experience required; first-time travelers considered - certifications (BLS AHA) - RN State license. Responsibilities: - Coordinate patient care and discharge planning in an acute hospital setting - Assess medical necessity per evidence-based guidelines - Develop and update patient care plans - Manage disease and needs assessments, including ordering DME - Ensure compliance with CMS, CPT coding, Department of Health, DRG, and HIPAA regulations - Review admissions and collaborate with clinical teams for optimal patient outcomes - Document accurately in Epic system - Work with specialty areas such as ICU, MS, SDU/PCU/IMC/Obs AS About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
