Provides counseling, support and assistance in making transition plans. Provides direction for routine aspects of non-medical problems of patients and their families. Assists with the planning and post-discharge of patients with outside agencies, nursing staff, and relatives. Requirements Required for Onboarding Core Mandatory Part III
