A company is looking for a Telephonic Care Navigator - Remote in

Key Responsibilities
Completes telephonic outreach to members and educates them on gaps
in care
Assists members with social determinants of health and links them
to community resources
Ensures members have access to primary care providers and meets
performance metrics
Required Qualifications
High School Diploma / GED or equivalent work experience
Must be 18 years of age or older
1+ years of call center or telephonic customer service
experience
1+ years of experience in an office setting using telephone and
computer
1+ years of healthcare, insurance, or community outreach
experience
